How to keep muscle while losing weight on a GLP-1
Rapid weight loss always costs some lean mass; how much depends on two things you control — protein intake and resistance training. Eating enough protein and lifting something heavy two or three times a week is the difference between losing mostly fat and losing a meaningful amount of muscle.
Why this matters more on a GLP-1
Weight comes off fast when appetite is suppressed. Trials of GLP-1 medication have consistently shown that a share of the loss is lean tissue, not fat. Losing muscle lowers your resting energy use, which makes the weight easier to regain later, and it affects strength and balance as you age.
Two levers, in order of impact
- Protein: aim for roughly 1.2–1.6 g per kg body weight per day, spread across meals.
- Resistance training: 2–3 sessions a week, covering the big movement patterns.
- Rate of loss: slower is generally more muscle-sparing — but appetite often sets the pace here.
- Sleep and steps: both support recovery and help you keep training.
A minimal strength plan that works
| Pattern | Example | Sets × reps |
|---|---|---|
| Squat / leg press | Goblet squat or machine leg press | 3 × 8–12 |
| Hinge | Romanian deadlift or hip thrust | 3 × 8–12 |
| Push | Push-up, dumbbell or bench press | 3 × 8–12 |
| Pull | Row or lat pulldown | 3 × 8–12 |
| Carry / core | Farmer carry or plank | 3 × 30–45 s |
Two sessions a week covering all five patterns is enough to send a muscle-retention signal. Add weight or reps over time.
Signals to watch
- Strength dropping on the same lifts for several weeks.
- Clothes fitting looser but body weight falling faster than ~1% per week.
- Constant fatigue, poor sleep, or hair shedding — often under-eating.
- Feeling unsteady on stairs or standing up from a chair.
What not to do
Do not add long daily cardio on top of a suppressed appetite. Cardio is fine for heart health, but it does not protect muscle and it competes for the little energy you have. If you must choose one thing, choose protein plus two lifting sessions.

Frequently asked questions
Can I build muscle while losing weight on a GLP-1?▾
In some cases, especially if you are new to training and eating enough protein. More realistically the goal is to keep most of what you have while fat comes off.
Is cardio useless then?▾
No — it is good for heart health and mood. It just is not the main signal for muscle retention. Keep it moderate and do not let it crowd out protein and lifting.
How fast is too fast?▾
Losing more than about 1% of body weight per week for several weeks is a flag, because lean mass tends to make up more of that loss. Discuss the pace with your clinician.
Do I need creatine or other supplements?▾
Creatine is well studied for strength and is generally safe in healthy adults, but it is optional. Protein and training do the heavy lifting; supplements are a small addition.
